Referrals to BHN
We are currently accepting referrals across the majority of our allied health, mental health and children’s services. To refer your client to Better Health Network, please complete the BHN referral form.
Referrals for services funded through South Eastern Primary Health Network (SEMPHN)
To refer your client to a service delivered by BHN and funded by South Eastern Melbourne Primary Health Network (SEMPHN), please visit the SEMPHN website and search for the program name. This includes Mental Health Integrated Complex Care (MHICC) and Residential Aged Care Wellbeing Program (RACF Wellbeing) and Accessible Psychological Interventions (API) amongst other services BHN provides.
Referrals for residential aged care wellbeing
For Residential Aged Care Facility (RACF) Wellbeing referrals please download and complete the Residential Aged Care Wellbeing Program Referral Form and upload the completed referral to SEMPHN.
