Care Finder

Care Finder offers practical support to help older, vulnerable adults access aged care services and supports including navigating My Aged Care. It is an intensive support service for people who have no family, friends or carers to assist them, to set up support services.  

How Care Finder can support you 

Our care finders take the time to get to know you, understand what matters most in your life, and support you to access services that meet your needs.  

Care Finder can help you find and access aged care and other local services by: 

  • Explaining aged care services and My Aged Care pathways.
  • Helping you register with My Aged Care and understand what services you can access.
  • Finding and connecting you with local support, including home care, health services, respite, and community support.
  • Helping complete forms, follow up referrals, and coordinate next steps.
  • Liaising with service providers and advocating on your behalf.

Accessing Care Finder 

Care Finder is designed for older people who have limited support and may need extra help engaging with aged care and community services. 

You may be eligible if you are: 

  • 65 or older, or 50 and over for Aboriginal and Torres Strait Islander peoples.
  • Frail or experiencing premature ageing.
  • Socially isolated or have no family, friends, or carers able to assist.
  • A vulnerable person who would benefit from ongoing, intensive support.
  • Experiencing barries accessing My Aged Care or services, including: Communication or language barriers.
  • Difficulty understanding or processing information.
  • Reluctance or anxiety about engaging with government services.
  • Safety concerns or instability in their living situation. 

Service locations 

We can work with you face-to-face in a place that feels comfortable for you. 

The Care Finder Program currently supports people living in: 

  • City of Port Phillip 
  • City of Stonnington 
  • City of Glen Eira 

From 1 April 2026, the program will expand to include: 

  • City of Bayside 
  • City of Kingston

Cost 

Care Finder is a free, government-funded service delivered by Better Health Network and commissioned by SEMPHN.
