Internal Referral Form

A rapid referral recognises that a client may need a sooner appointment due to identified risk factors, including concerns related to safety, mental health deterioration, extreme vulnerability, or the potential for worsening health outcomes without timely intervention.

An interpreter is suggested if the client doesn’t speak or understand English well enough to communicate clearly and provide consent.

A supported handover ensures the client is not left to navigate services alone, with the referring service supporting the transition to the new service.
