Dementia Support Services

People living with dementia, their families, and/or carers are supported by the BHN Dementia Support Service.

About the Dementia Support Service:

  • Assists people with cognitive decline and/or dementia to remain living independently at home.
  • The service is staffed by nurses and occupational therapists.
  • Support is provided face to face at home or in the community.
  • We focus on wellness, reablement, and restorative care approaches and support processes for advanced care planning.
  • The service has access to other specialist medical and allied health services to provide coordinated
    care.

The service provides: 

  • Comprehensive assessment of physical, social and emotional wellbeing
  • A team working with you to develop a care plan with a focus on strengths, independence and interests
  • Assistance and support to link to a range of services
  • Transition to other services and supports in your community
  • Ongoing monitoring depending on need.ie. social supports and respite options
  • Our service aims to assist people with cognitive decline and/or dementia to remain living independently at home.

  • We focus on wellness, reablement, and restorative care approaches and support processes for advanced care planning.
  • The service has access to other specialist medical and allied health services to provide coordinated care.

Services for carers:

  • Advice on managing behavioural and psychological symptoms of dementia
  • Advice on managing continence, safety around the home, community and general health
  • Education through individual and group sessions
  • Group sessions include men’s and ladies carers lunch, a monthly concert and lunch for both carer and clients.
  • In addition, we run a weekly walking group and monthly movie group for clients.

Who can use the service?

  • People who have cognitive decline and/or dementia
  • People who are eligible for a Commonwealth Home Support
  • Programme (CHSP)
  • People living in the Kingston and Bayside LGA’s
